Services - DOT Physical Exams
Dr. Armani is certified to provide you a Department of Transportation (DOT) commercial driver’s license (CDL) physicals and substance abuse exam. She understands what the DOT medical exam requires and will have all the necessary forms for you to fill out during your visit. However, if you wish to speed up this process, please feel free to download this updated DOT form and bring it to your appointment with your information and health history already completed.
